Improvement in function, hospitalization rate, emergency use, and other clinical and process measures. CMS publishes the exact list. How to improve
Pull your report and see where you’re below average. Focus on one or two measures: better documentation, care coordination, and follow-up often move the needle. You can’t fix everything at once.
