State licensure is the first gate. Every state has its own rules: application forms, fees, proof of insurance, policies, and sometimes a site visit or orientation. Missing one item can add weeks,or months if you’re in a backlogged state. Experienced operators treat the state checklist as the single source of truth and never assume “close enough” is enough.
What varies by state
Some states require a physical office in the service area; others allow a central office. Some want a director of nursing named before they’ll process the application; others allow a conditional approval. Pre-application meetings or orientations are common, check the state health department’s home health or home care page and sign up. They’ll tell you exactly what they want and in what order. States that offer optional pre-application calls or site visits are worth using: catching a missing policy or wrong insurance limit before formal submission can save you 4–8 weeks of back-and-forth.
How to avoid delays
Get the full checklist from the state and tick every item, our essential policies and procedures before your first survey guide lines up with what surveyors ask for. If they want policies, send policies, not a table of contents. If they want proof of liability insurance, send the certificate with the right limits (states often specify minimums; going under will get you rejected). If they want an org chart, send one with real names or “TBD” and a timeline. Follow up if you don’t hear back within their stated timeframe; one polite call can unstick an application. Managers who’ve done this in multiple states keep a state-specific checklist so nothing is forgotten when they expand.
Renewals and multi-state
Licenses expire. Set a calendar reminder 90–120 days before renewal so you have time to gather updated policies, insurance, and fees. If you operate in more than one state, each has its own cycle, track them in one place. Letting a renewal lapse can trigger a survey or suspension. A single checklist template that you customize per state (with due dates and key contacts) keeps renewals from slipping.
