What to review before OASIS lock
Key items: M0090 (SOC date) correct; M1020 series (diagnoses) aligned with physician order and face-to-face; no contradictions with visit notes or POC. Completeness: required items answered, not defaulted or left blank. Consistency: same clinical story across OASIS, order, F2F, and notes. One bad OASIS can affect the episode payment and your quality reporting.
Who should own it
Assign one person (e.g. clinical lead or designated reviewer) to run the pre-lock checklist before every SOC and recert OASIS lock. No lock until the checklist passes. If your EHR supports it, build the check into the workflow so it can’t be skipped.
Cost of skipping
