Orders and updates
Any change in frequency, discipline, or diagnosis needs a physician order. Keep the order-to-note alignment so plans and orders don't drift. Don’t assume a verbal okay is enough; get it in writing. Send the plan of care (or updates) to the physician for signature within the required time. If they’re slow to sign, build in a follow-up process so you’re not at day 30 with an unsigned plan.
Communication
Report significant changes: new symptoms, hospitalization, or patient/family concerns. Document that you notified the physician and what they said. Some agencies use a simple log or template so nothing falls through the cracks.
